Original equivalency certificate (issued by the university).

Copy of valid National ID card, stamped by the university.

Personal photo of the pharmacist, stamped by the university.

Transcript showing success in the “Pharmaceutical Legislation” course (from university).

Certificate of good conduct (from the university).

Certificate of 800 hours of training, stamped by the pharmacy + copy of the pharmacy license (not stamped).

Two (2) copies of the pharmacist’s birth certificate.

Copy of pharmacist’s high school certificate.

Recent criminal record certificate addressed to the General Syndicate of Pharmacists.

Two (2) copies of military status (males only).
